2012 National Survey of Science and Mathematics Educaiton: Science Program Questionnaire

The audience for the instrument is school science program heads. It solicits information regarding courses offered at the school, resources committed to science instruction, and factors affecting instruction. The questionnaire was developed and administered to a nationally representative sample of schools as part of the 2012 National Survey of Science and Mathematics Education. Program questionnaires were customized according to grade levels included in each school. Web-based administration allowed customization of surveys “on the fly,” such that respondents were not presented with irrelevant items.
